What Is Root Canal Treatment?
Root canal treatment is a restorative endodontic procedure that prevents tooth extraction in cases where the pulp layer — the innermost part of the tooth containing living tissue — has become infected or damaged. Also commonly known as “nerve removal,” root canal treatment preserves the natural tooth structure and maintains the tooth’s chewing function.
In modern dentistry, root canal treatment has become a painless procedure with high success rates, thanks to advanced imaging systems, nickel-titanium (NiTi) files, and apex locator devices.
When Is Root Canal Treatment Needed?
Root canal treatment may be recommended when one or more of the following conditions are present:
- Deep tooth decay that has reached the pulp
- Damage from cracks or fractures that has reached the nerve
- Abscess or infection developing at the root of the tooth
- Prolonged and severe sensitivity to hot and cold stimuli
- Spontaneous toothache that worsens at night
- Noticeable darkening or discoloration of the tooth
- Pulp damage following repeated filling procedures
- Teeth that have lost vitality due to trauma
Some of these symptoms can progress to tooth extraction if treatment is delayed. Early diagnosis is therefore critical.
Stages of Root Canal Treatment
At Dental Plus Istanbul, root canal treatment stages are planned in accordance with modern endodontic standards.
1. Examination and X-Ray Assessment
At the first appointment, a clinical examination of the tooth is performed. A periapical X-ray is used to evaluate the root structure, number of canals, and the extent of infection. 3D dental tomography may also be used when necessary.
2. Local Anesthesia
The treatment area is completely numbed with local anesthesia, ensuring the patient feels no pain during the procedure. Treatment begins once the anesthesia takes effect.
3. Rubber Dam Isolation
The tooth being treated is isolated from the surrounding teeth using a special rubber sheet called a rubber dam. This keeps the working area sterile and directly improves treatment success.
4. Access to the Pulp Chamber
A small access cavity is made through the chewing surface or back surface of the tooth to reach the pulp chamber. All decayed tissue is thoroughly removed.
5. Cleaning and Shaping the Canals
The infected nerve and vascular tissue is cleaned down to the root tip using specialized endodontic files. An apex locator device measures the true length of the canal with millimeter precision. The canal interior is disinfected with solutions such as sodium hypochlorite and chlorhexidine.
6. Canal Filling (Obturation)
The cleaned and shaped canals are filled in a sealed, airtight manner using a biocompatible material called gutta-percha along with canal sealer. The quality of the filling is verified with an X-ray.
7. Tooth Restoration
Once root canal treatment is complete, the tooth is restored with a filling or crown. Particularly for posterior molars, a zirconia crown or onlay is commonly recommended to protect against the risk of fracture.
Does Root Canal Treatment Hurt?
Contrary to popular belief, root canal treatment does not cause pain — in fact, it eliminates the source of existing tooth pain. Thanks to local anesthesia, patients may feel only mild pressure during the procedure; no pain is experienced.
Mild sensitivity or discomfort while chewing may occur during the first 24–72 hours after treatment. This sensitivity:
- Is easily managed with pain relievers recommended by the dentist
- Resolves on its own within a few days
- Is generally a normal part of the healing process
Severe, persistent, or prolonged pain lasting more than a few days warrants a visit to the clinic.
How Long Does Root Canal Treatment Take?
The duration of root canal treatment varies depending on the location of the tooth and the number of canals:
- Front teeth (single canal): 30–45 minutes, completed in one session
- Premolars (2 canals): 45–60 minutes, completed in one session
- Molars (3–4 canals): 60–90 minutes, in two sessions if needed
In cases involving an abscess or severe infection, the number of sessions may increase. Thanks to modern endodontic techniques, many cases are successfully completed in a single session.
What to Keep in Mind After Root Canal Treatment
To speed up recovery after treatment:
- Avoid eating or drinking until the anesthesia has fully worn off (risk of oral burns)
- Do not chew hard foods with the treated tooth for the first 24 hours
- Limit smoking for the first 48 hours (it negatively affects healing)
- Take prescribed pain relievers and antibiotics regularly
- Continue brushing as normal; clean the treated area gently
- Avoid putting excessive pressure on the tooth until the permanent restoration (filling or crown) is complete
Success Rate and Tooth Longevity After Root Canal Treatment
When performed with the right indication by an experienced team, the success rate of root canal treatment is around 90–95%. A tooth that has undergone root canal treatment can be used for a lifetime with proper care and appropriate restoration.
Key factors affecting treatment success include:
- The severity and duration of the infection
- The quality of canal cleaning and filling
- Timely and appropriate placement of the permanent restoration
- The patient’s oral hygiene habits
- Regular clinical check-ups
What Happens If Root Canal Treatment Is Not Done?
When treatment for a tooth that requires root canal therapy is postponed:
- The infection can spread to the jawbone, forming an abscess
- Severe and persistent pain develops
- Facial swelling and fever may occur
- Permanent damage to the tooth and surrounding bone tissue can result
- Ultimately, tooth loss may become inevitable
For this reason, it is of great importance not to delay a recommended root canal treatment.
